Family Case Management is a program funded by the Illinois Department of Human Services. There are no income guidelines for these services which are available to all residents of Fulton County. The goal of the program is to improve prenatal care and reduce infant mortality rates in Fulton County . Case management activities may include information and referrals on: (1) prenatal care, (2) infant growth and development, (3) Immunizations, (4) well child care, and (5) accessing appropriate health care and social services. Home visits and follow up on services delivered are also provided. All case managers employed by the Fulton County Health Department are Registered Nurses licensed by the State of Illinois. Training and conferences in maternal-child health care programs are available to our staff so that they remain current on the latest health care issues.
