       WIC Nutrition Program
WIC (Women, Infants and Children) is a U.S.D.A. funded food supplement program for Fulton County residents who meet financial and medical or nutritional requirements.

WHO QUALIFIES FOR THE WIC PROGRAM?
Pregnant women, nursing women, infants, and children under five years of age. Income eligibility for the WIC program  from July 1, 2005 through June 30, 2006 is based upon your gross income is and family size.

FAMILY SIZE: All members living in one household
GROSS INCOME : Wages, salaries, unemployment compensation, Social Security income, etc.
You are income eligible if you, the applicant, receive any of the following program services:
a) Public Assistance
b) Medicaid
c) Free or reduced school lunches
d) Food stamps
e) Weatherization assistance
PREGNANT TEENAGERS: Are considered on an individual basis.

You will receive food coupons for the following available foods:
* Milk and cheese, cereal, fruit juices, eggs, dried beans. (Pregnant, Breast -feeding Women and children)
* Formula with iron, infant cereal, infant juice. (Infants to one year)
In addition, you will receive nutritional assessments and education. Also, you will be eligible to receive any of the health services of the department, including immunizations and physical assessments.

Please bring the following with you to each certification appointment: Identification (Driver's license, photo ID.); Proof of pregnancy (Positive pregnancy test); Proof of income (Your last two paycheck stubs, income tax form, Medicaid or KidCare card or food stamp ID); Proof of residency (Your utility bill or recent mail); Proof of birth (Birth certificate for your infant or child); Proof of immunizations (Shot card); WIC does not require proof of citizenship or alien status.

WIC Certification Clinics are held on Mondays, Wednesdays, Thursdays.
WIC Classes are held on Tuesdays

Call the Fulton County Health Department at 309-647-1134 ext. 254 between 8:00 A.M. and 4:00 P.M. or call the Astoria Sub-Office at 309-329-2922, Monday-Wednesday, 8am-4pm.   An Equal Opportunity Employer
