Mission Statement
The Community Health Action Team exists to address the health needs of Fulton county residents through community
collaboration for needs assessment and planning, promoting health awareness and prevention measures, and establishing measurable outcomes for implementation activities.
CHAT Goals
The Community Health Action Team is committed to building healthy communities in Fulton County through the
following goals:
1.
To develop plans that address current health needs in order to improve the future health status of county residents.
2. To obtain active participation of individuals representing county agencies, businesses, organizations, schools,
and communities to address identified health needs.
3. To provide health education, promotion, and prevention measures that enable county residents to accept personal
responsibility for their health.

The Achievement Report for the Community Health Action Team (CHAT) describes activities conducted through the
collaboration of county agencies. The CHAT organization was formed in 1995 through a partnership between Graham Hospital and the Fulton County Health Department. Partners contribute resources towards the
salary of the CHAT Coordinator and costs associated with the planning and implementation of activities. In 2001, the Fulton-Mason Crisis Service joined CHAT as a partner to address health and social needs. In
2004, CHAT received a federal grant from HRSA to develop and website and webportal for information and service referrals.
